Default BR Strategies

First of all I want to thank the administrators of this site for providing it - I just arrived and I plan on spending many an hour here. I have a couple of BR and related questions but before I get into them - a quick background on me might be helpful:

I am just getting back to poker after a nearly two year forced by life and circumstances haitus. (A financial disaster unrelated to poker and family illness in a non-gaming state) I've spent two years thinking, studying and dreaming about making it back to the tables. I found that I was quite sloppy in many aspects of my game - and resolved not to allow that to happen again. I developed a hunger and obsession for playing during that time I couldn't that probably should have me calling Dr. Schoonmaker for an appointment - but it'll have to wait. In the past, amongst other failings, I didn't study or keep records - not this time. I made it back to the poker tables 3 months ago with a vengence. Ok, enough about me...on to poker.

I found my way to a casino promotion which offered cash rewards for hours played which seemed like a good way to kick off by BR. Play 900 hours and recieve $5000. They sweetened the deal by offering my other half a job thereby allowing me to concentrate on bankroll building instead of bill paying. This time I've kept accurate records. Though I've only been playing again for 3 months - I've put 1000 hours in at the tables - and while not the 2000 hours I would like to reduce the SD - enough of a sample to arrive at some conclusions. Seems I recall David Sklansky did an article in a defunct magazine called Poker World [?] comparing expected win with what I believe he called 'max swing'. I'm assuming that I could use my win rate to date (based on 1000 hours) along with the formula he offered to predict an accurate range of expectation for the future. That's Question #1: Anyone recall the formula or have a copy of the magazine and can pull it up for me (I believe there were only two or three issue's)?

Question #2: Is it correct for me to assume that the large number of hours in such a short time influenced my EV negatively to such a degree that I should add the $5000 to my wins - or should calculations be done ignoring the $5000?

Question #3: I'm not sure how to phrase this question so allow me to ramble for a bit. My goal when I returned to poker was to eventually return to the $40/80 level when my BR could sustain that level - I've begun to question that. I've made such tremendous gains in BR health at the lower limits (3/6, 4/8, 6/12 and 8/16) these last few months that now that I am able to move up comfortably to 15/30, 20/40 or 30/60 - I'm questioning the wisdom of the move. The casino I'm closest to has games up to 20/40 (no 15/30 however) but I'm an hour or so away from a larger and wider range of games. It's pretty hard to play winning poker and lose much in a 6/12 game even if the short term luck thing is going every way but yours. On the other hand, finding myself in a 30/60 game with tougher opponents (re: lower expected win rate in relation to game size) when things go poorly can be demoralizing to one's psyche and BR. So when do you move up? What are some of the questions you ask yourself before you do?
