I know it's probably not what you want to hear right now, but is 5th normally a fold? (Dead Q, some dead spades).

it looks like villian picked up a pair of jacks on 4th (he would have likely completed 3rd with kings are jacks in the hole). SO hero's queens are likely best at this point. However, when villain bets out on 5th hero should be a little worried he's against a straight draw (I know that 3 queens are accounted for, but what else would he decide to bet out with unless he's betting a gut draw?)

Ouch. One question, though... unless you think he's 3-betting a straight, what value do you see in capping the river?

I agree with this; I'm just calling after he reraises. The rest of the hand seems fine. I don't think you should fold at any point.
