Default Miscues during hand play

How many of you have seen this or done this. Can this be converted to an angle-shooting?

Player A is to act and says "Bet", but hasn't indicated an amount yet, Player B immediatly says "Call". Obviously Player B isn't obligated to call, at least according to one floor decision I've seen, who's reasoning was: "Player A turn doesn't end until he completes his action, which would be actually putting a bet in the pot, therefor player B announced call was out of turn and not binding until and unless an amount is indicated". So had player A announced "Bet $500", then Player B announcing a call would be considered valid action in turn.

I've also seen Player A announce "Bet" and Player B announce "Raise" -- but it was uncontroversial because they both held the nut straight so neither made an issue of it.
