MTT Results Spreadsheet?

Thinking about getting serious about tracking my MTT play as my roll is getting to a point where I can play them regularly. I'm don't see one in the archives, but wondered if there was a decent results tracking spreadsheet out there? (I play on PP mostly, and can't get them to send tourney summaries to PT)

They're pretty easy to make. These are my categories:
Date. Time. Site. Game. Buyin. Entry Fees. R/A. Spent. Prize Pool. Payout/Person. Entrants. Place. $ Won. Net. ROI.

Game would be NLHE for example
Payout/Person is Prize Pool/People (only changes for rebuys)
Net is $won-$spent
ROI is Net/$spent
