As someone who might be innocently mistaken for a "lady" by someone unfamiliar with me personally, I think there's nothing more offensive than a man proffering a woman's opinion for her in the name of "chivalry." Foul language doesn't usually offend me (unless it's excessively suggestive/aggressive and directed toward me), but I have a brain and a mouth and can speak up for myself.
In the case of these "Derrrr... There's a lady present!" guys, who are usually 1.5-2x the age of their objective, my female intuition says all they're really trying to do is get that woman's attention directed toward him! I think the throbbing biological urges at the root of this lame attention-getting gambit are far from what we may think of as "chivalrous." The wife is a ball-busting divorce lawyer who expects no special treatment. She has no problem cussing up a a blue streak, and is not offended by others cursing. She does not expect others to hold the door for her, and when we were dating during college (the Reagan years), she refused to let me pay for anything.

I don't generally curse in front of women, but when I see them in a poolhall (I've never been to a casino), I generally don't bother watching my language. The poolhall was there before the women were. Most women curse a lot more than me, anyway.
