I think the pre-flop action is messed up. Did you limp re-raise, or just limp call?

I think he limp/called, either way he's losing his chips, if he reraises, CO is either pushing or calling with the Queens, he has a high pocket pair and position on hero. This is one of those hands you just can't win because villan hits his set.

I probably slow down on the turn when he calls your re raise. I think at this point you have to put him on AQ or a set. Check the river and depending on your read (AQ vs set) call or fold.

Then I really don't like this line. I think that you either need to open-raise from EP or limp reraise. I think that this is an OK line if you're 100% positive that you can outplay your opponents post-flop.

I think that you really need to raise this flop in order to define the villain's hand. After that, the whole hand plays differently.

If you'd been a bit more aggressive, you would've wound up with more information about the villain's holdings.

Villain played great. Flat calling the turn raise is excellent.

I might have bet more on the turn if I was villain, but I like it.

Of course, if Im hero - I hate the results, but I have to be more than a little worried about "bet less than half pot but call a check-raise" !!
