i lived with a guy whose last name was REEFER. spelled like that and everything. he said in grade school people would make fun of him by calling him "beefer" because they didn't realize his last name was a joke unto itself. best off the top of my head is beefer.

There was a huge, beastly chick in college known as Chewbacca. Her appearance didn't stop (at least) two of my friends from having sex with her.
We had it on good authority that a friend's ex-girlfriend had a particularly embarrassing, chronic STD. Her nickname? Dirty South. One guy called her this to her face at a bar and got clocked for his trouble.
Also there was a guy that nobody particularly liked who we referred to as Fatty McButterpants. This nickname was so popular that few people even knew his real name.
