Anyone do web design and scripting?

I can whip something up in CGI/Perl. My programming is good, but I'm no designer.

It matters quite a bit what you want done with your submitted form. I have done quite a bit of Internet development, primarily in Perl.

basically i just want 10 or so fields and a few checkboxes to be filled out, then a submit button which will translate those fields into a legible document and email it to me. Should be very simple (I think).

Very simple to do in any language I'd assume. Just dependent on personal preference and server support.
