Carson contends that the book actually was written by Miller.

My apologies for not being clear. In my mind, I was thinking that the discussion at this point was about SSH and the surprise at the number of citations to Carson's book that appeared in SSH, given the apparent conflict between Carson and the other two authors on SSH.

I agree, in fact I believe in some ways it laid the foundation of SSH. They better at least footnote it!
