Back to Basics: Value extraction vs a Maniac

I'm on a somewhat unpleasant downswing (-75bb, yeah I've had worse but it still sucks), and I'm starting to suspect that I'm tilting. So I'm just gonna run this by real quick to make sure I'm not turning into a fish. Villain is an honest-to-God maniac, something like 60/24/6.0 or so. His first hand he 3-bet preflop with KTo.

So the Manual for Maniacs demands that I simply check/call this down, but this is a pretty icky flop where spiking an 8 may or may not help me, especially if it's a heart. Call down anyway? And does anyone object to a c/r on the river?
