Default Small Stakes Hold\'em

In the book Small Stakes Hold'em there is a very handy starting hand page for both loose and tight games. It shows you which position to play certain hands etc, very good.

Here's an example of the what hands to play in middle position:

Middle Position (three seats left of early position)

If There is no Raise, Play:

AA KK QQ JJ TT 99 88 77 66 55 44 33 22
AKs, AQs, AJs, ATs, A9s, A8s, A7s, A6s, A5s, A4s, A3s, A2s
KQs, KJs, KTs, K9s
QJs, QTs, Q9s
JTs, J9s
T9s,
98s
AK, AQ, AJ, AT
KQ, KJ

Raise With: AA, KK, QQ, JJ, TT, 99
AKs, AQs, AJs, ATs
KQs, KJs
AK, AQ, AJ
KQ

Against a Raise: Call with: AA, KK, QQ, TT, AKs, AQs, AJs, KQs and AK

Re-Raise with: AA, KK, QQ, TT, AKs and AK

Against a Raise and A Re-raise: Play and Raise with: AA, KK, QQ, AKs

As you can see from above they mention when to call, raise or re-raise depending on who's raised in front you. The thing they don't mention, what if a few players limp in front of you..??

For example it says to raise with KQoff in middle position, now does that mean with no limper's??? how do you play KQoff in middle position if 3 players have limped in front of you??

I can't understand why he didn't mention this in more detail, perhaps I've missed something or am just being dumb as usual?
