Default Freezeouts

Hi,

Ok, so I have my heads up opponent all picked out and ready to play a heads up freezeout in limit hold 'em. But what should the details be, to maximize EV? Specifically:

How many BB should we each start with? (I know it probably depends on my variance, but an approximation would be appreciated.)

What blind structure? (i.e. 1-2 or 3-6?)

Should the blinds increase with time, or hands played, or neither? If so, by how much should they increase?

Assuming I can generally outplay my opponent regardless of his playing style, should any of these details be a function of my opponent's playing style?
