Default starting record keeping questions

one of my new years resolutions is to take my poker hobby a little more seriously, and really try to improve my play.

my first step is to start keeping records, I created a crude excel spreadsheet to keep track of some of these things, but then realized someone probably has already done this.
does anybody have a good spreadsheet they use?

what are the most important things to track? right now i've got ROI (a whopping 50% right now, after a whole 10 games) and ITM%

I'm playing mostly $5 games, what should be my benchmarks as far as stats and bankroll on when it's time to move up?

any other tips you can give to a rookie?

Use this spreadsheet and expand it if need be. I have been using the previous version and it makes tracking your ROI/ITM, and a few other things, very easy.
