Default Preferred Player Application at Party - Some Questions

A few questions:

If I fax these today, Dec. 17, will they be confirmed in enough time to get the $200 bonus before the 22nd? If not when can I expect them to contact me?

Do I have to fill out a seperate document for each skin?

Is this the accomplishing the same thing as becoming a VIP Player, a VIP Gold Player, and a VIP Platinum Player?

I would email support but they suck.

Don't know, probably not since they're slow as hell over there.

Yes, even they all use the same cashier system I had to jump through the same hoops everytime I want to use my checkings account a Party skin even though my account's already on file.

Could someone possibly direct me to this website then (The one for Party/paypro). I cannot find any mention of it anywhere except in the Paypro FAQ.
