Rebuy-Add On Suggestions

We are in the process of going to a Re-Buy, Add On format for our small home game. There are a few of us that have participated in B&M games that had this format, but they are all just a little different in pricing structure as well as chip amount for price.
Would like to get an idea of the 'Norm' for setting up this format to be fair and equitable to all players.
