Depends. If the limper is loose or BB is tight, yes. If the limper is tight passive PF and BB is loose. I could see a complete, but mostly raise it because you'll have the limper in a bad way a lot and you want BB's dead money in the pot.

I'd raise all of them. If not, then sb because of position.

I raise em all, except for that some of the time I fold it UTG depending on the way the table is playing and my image. KQ is an automatic raise for me but KJo is a bit too weak. This might be too tight since everyone here seems to think it's a standard raise.

I raise all four. I used to just complete w/the last two and try to flop a pair but I think it's better to push the preflop edge even at the sake of increased variance.

Standard raise. It's more difficult imho when it's raised before you.
