I'm not Stew, but hopefully my answer will suffice! No you don't need to fax your life details but they do require you to cashout by using a PIN that they send to you via snail mail. It takes about 3-4 days if you live in the UK, I think its 8-10 for the US.

The main reason I don't play pontoon is that I don't know how to play it to the best advantage.
I have also heard the horror stories from those who have played it. Play it perfectly and yes the house advantage is only .17%, but why is it that all the stories I have heard always talk about how bad the beats are? I have never heard of any stories where the player actually won money playing pontoon. I have been thinking about trying the game. I thought about depositing 100 dollars without a bonus and giving it a go, then reporting the results here. But I still haven't even looked at the Wizard's strategy for it yet because I don't have a clue on how to play it.

Depositing 90, getting a 90 bonus and cashing out 700 while never varying my bet size from $5 has happened in Pontoon. In the year prior to them introducing Pontoon I never more than doubled my bonus on BJ.
The swings are huge, but the HA is small. Giving up EV for lower variance is a silly thing to do. www.wizardofodds.com has a pontoon chart for perfect play.
