So has every possible permutation of cards and player actions occurred? To what degree of accuracy do we know this to be or not to be the case?

Given that I don't know how many poker hands have been played and how many possible permutations of player actions there are...well, I guess that's why I don't realize that it's not even close.

Maybe we could figure out which ones haven't occured yet and make them happen. ;-)
