basic terminology question

this is a very basic terminology question.

when i read about NL hand and someone says they raised 200, do they mean "they raised TO 200" or "they raised BY 200".

is there an assumption we should make? i.e. a custom... or do you have to ask the person? and they should have made it clearer.
