Default QQ through 10-10 OOP Help Needed

I have been struggling lately with the mid-high pocket pairs, especially when OOP. I am trying to determine whether it is a bad run or if I am playing them poorly.

I typically play $1-2 6-max, sometimes full ring. I also dabble in $2-4 full ring.

Here are my questions:
Preflop
1. Do you raise these hands preflop in EP or from the blinds? If so how much?
2. In EP if you raise and someone comes over the top, what is your strategy?
3. Do you re-raise an open raise from the blinds?

Flop (again assume OOP)
1. If you are the aggresor, and undercards come out, how do you handle the pressure of someone playing back at you (assuming you preflop raised)?
2.If you just called another's preflop raise and undercards come out, how do you proceed?
3. If you are p/f raiser and an Ace or King hits the board what is your strategy?

Assupmtions
1. Average non tricky opponent, with no good read (I know this is player dependent).
2. You are always OOP.
3. Everyone has 100 BB
4. 1 or 2 opponents are along for the ride.
