I am about to provide the best response you deserve to get to this post and you're most likely going to get flamed pretty good for such a well-posed question.

Look in the indian reservations.

Turning Stone: Verona, NY
Canterbury Card Club: Minnesota

Also, some Indian Reservations (but not all, call first). The Connecticut Indian casinos are 21+.

Turningstone

Also any in Canada (other than Alberta or Quebec) will be +19. In Alberta and Quebec they would be 18+.

Michigan Indian Casinos are 18, call to be sure but I know for a fact that Soaring Eagle Casino in Mt.Pleasent is, they spread almost every game you can think of. Leelanau Sands in Travese City is also 18

Some Indian Casinos in California are 18+ as well. Morongo, Barona, and Santa Ynez come to mind. I've played at both Morongo and Barona and both are run rather efficiently and the facilities are top-notch IMO. You just cannot check into a room at either if you are 18-20, you must be 21. Other than that everything else is fair-game (alcohol aside).

Morongo is where the recent Poker SuperStars Invitational Tournament was held.

The Bahamas is 18+, but poker is illegal.
Most cruise ships are 18+.

Bahamaians cannot gamble, period. Poker is illegal in the Bahamas, unless you get a permit from the Government, like the WPT did. Surely, the country's second largest employer, Atlantis, could lobby to legalize it, but why should they? Poker doesn't make enough to cover the rent on Paradise Island.
