Default Note taking during Live play(not online)

am currently in a sustained downswing, and I am in the process of trying to fix my leaks. At this time the only $$ games I play are live. I play online at PSO. Havin said all that, I get to the point.

DOES ANYONE HAVE A BASIC TEMPLATE FOR RECORDING CARDS PLAYED IN A LIVE (NON-ONLINE) GAME? I HAVE TRIED A COUPLE OF DIFFERENT TIMES FOR SOMETHING USABLE, BUT HAVE YET TO FIND ANYTHING THAT WORKS.

PLEASE ADVISE.

What I am really looking for is basically an simple template that I can use. If anyone happens to have anthing along those lines please pmail me.

Thanks, I really didn't know where to post this. I mostly play in home games, so I didn't think B&M w/be a good forum. But I'll try that too, thanks
