Given the way he's played this hand, I am assuming UTG is LP. Hence I would bet/fold the river, hoping for some free value from him to compensate for the trouble we might be in from SB.

AK and AQ would have check raised you by now. TT or JJ had odds to call the flop and turn with the gutter and he may look you up. AT or A9s seem possible especially four-handed. UTG likely has hearts, but maybe a weak ace or king that will call, and he probably isn't going to bluff at the river. I think you need to bet the river.
