Default Reads

As we all know, reads are of the utmost importance in SH play, much more important than they are in full ring play.

However, I see many hands posted that have no reads, or just stats as the reads (I too am guilty of this sometimes).

For those of you who are proud of your player reading skills, what sort of things do you constantly take notes about? What is the average numbers of tables you play at once/number of notes per player? What notation do you think is best?

Anything else you see as relevant, add it in. I think a good discussion on this would be very helpful. If anyone has a few hands they played and the notes they made afterward, that would be excellent too.
