Default Unlucky or bad play Heads Up?

I've been finishing 2nd a lot lately. I thought some of it might be attributed to being a little passive too long and usually being behind HU, but then this:

[Note: it's Pot Limit. I'm finally switching over to NL because I have the BR to play 50s but they never fill up in PL]

Glenny35 (never played with him before) is a little loose and was caught bluffing a few times earlier. Therefore I don't trust him but haven't had the chance to catch him myself yet.

My image is tight and aggressive except for a hand I reraised a total lag with Q5s (which he called with Q8o) and got lucky to hit 2 pair to bust him.

Starting with first hand HU, I've left out a lot of hands where I folded PF or CCed then folded flop:

A bit of both.
You call/fold too much. Push or fold preflop.

P.S. don't fold.
