I have... I keep notes on books I read and I have all tables I need in there and also interesting summaries of articles I've read.

In terms of hands I play, I look through and take notes in PT. I haven't taken notes in my notebook about certain hands yet, but probably should.

How about different sets/formats of journals for online vs B&M play? Online, it's easy to dump specific hand info into PT or other tool, but live it's a different story.

I've seen a couple of people taking notes at the table, but I don't know what kind of info they are tracking. What kind of info would you deem important? Perhaps # of hands entered vs hands dealt, card info, reads on players? Sounds like a lot on info to track live. You could get carpal tunnel or something in a long session.
