Default Karlson-Sklansky Hand Rankings

I have done a 2+2 search and a google search (which generates almost exclusively 2+2 posts), but I am unable to find how the KS hand rankings were generated. I suspect there is an explanation hiding somewhere. Does anyone know where, or could it be explained here briefly?

Speculation:
They are based on mathematical-likelyhoods of being profitable hands given the likely metagame and handranges of the opponents
