Default Re-Raising before Flop

Hello,

According to Sklansky in Texas Hold'em for Advanced Players, you should be folding or re-raising to a single raiser

Is it ever correct to Cold Call short Handed?
What is a rough estimate of hands to Re-Raise with as a semi-bluff re raise pre flop if your opponent will come in raising with any 2 decent cards.
