Health Insurance Issues

Ok so i have a few questions for all you experienced OOT posters. I am getting individual health insurance after being on my parents insurance for my whole life. 1st i occasionally smoke cigs. Not habitually and not even 1 per day. Should i tell them i smoke? How much more expensive is it if i do? 2ndly i smoke weed daily and have for quite a while. Are they going to require a physical before coverage begins? Is a physical going to turn this up?

Any experience would be greatly appreciated.
