David's assertion in HEFAP is that in a game with a lot of extra blinds, you could easily make a sufficient profit with the extra action generated by your premium hands. This is no longer true if you are forced to post the larger blinds as well; your cost to wait for those cards is too high.

So his comment that you should play tighter, doesn't apply if you are posting larger blinds as well.
