Anyone who regularly draws to a runner-runner, cannot be a winner IMO. However, several weak draws plus some chance that you currently have the best hand can justify peeling a card on the flop, and it may appear to be a little fishy to an observer.

Its also important to remember that in a full game a gutshot is a gutshot. In heads-up, however, its a gutshot, a back door flush draw, and probably one or two pair outs as well. I think part of being an expert heads-up player is recognizing these situations.
