A good BJ incorporates a good deal of hand action (unless we're talking deep throat), and is, as someone else pointed out, essentially a glorified handjob with a little bit of mouth action mixed in. There's just not enough friction in a hands-free blowjob.

Wow. Glad I read this thread. I thought I was the only guy in the world who never got a good BJ. Yet for some reason, I remain convinced that she's out there, somewhere.
As someone said, 69 is best. Probably because it keeps her motivated--she doesn't start to act impatient or disinterested after just a few minutes.
