There are three things you should be keeping notes on for each player: CB, min-raise PFR, min-raise post-flop.
My notes are: CB:: STD, No = Miss, weak, or overcard (OC) - which would be QQ checking when an A or K flops, and lastly, CB:: STD + Ck = Miss or Weak. A lot of players will make a CB then check the turn if they get callers. And of course let's not forget CB:: No = c/r.

The point being it means different things to each player. Keep notes.
