Just having a maniac at the table isn't a good reason not to add-on. You want to make sure you have the all-in fool covered, twice over if possible. First off, because if you expect to double through him, you are actually buying twice as many chips- he can only double the stack you have. Secondly, you want to be able to survive a bad beat against the fool.

For me, it's how much I have invested at that point.

From an EV standpoint this makes no sense. Mathematically (that is, not taking skill into account) the only things that matter are your stack size, other stack sizes at your table, and average chip count. Money in the pot is no longer "yours", and this includes tournament prize pools.

One thing I have never seen mentioned in any of the books I've read, or on online discussions, is stack size relative to the blinds. In other words, if you have t10000 and can rebuy for t1000 more, would it make a difference if the blinds next round are 50/100 and if they are 200/400?

EDIT: I just realized part of the answer to my own question; it has to matter at least some because smaller blinds means more skill, and more skill means if you're one of the good players then the rebuy is going to have a higher expected ROI, meaning if the decision is marginal it could swing based on blind size. So the question now is how much does it matter?
