Default When was the last time we had a major dam break?

I saw the reports of a possible flood due to a dam break in Taunton, Massachusetts. I wonder, when was the last major dam break?

(a levy is not a dam, btw - New Orleans doesn't count)

Here is the report: (sorry for the ALL CAPS, that is the way it comes and I ain't gonna edit it)

...LIFE THREATENING SITUATION IN PARTS OF TAUNTON DOWNSTREAM OF THE WHITTENTON POND DAM ON THE MILL RIVER...

FLASH FLOOD WARNING HAS BEEN RE-SENT WITH SOME ADDITIONAL INFORMATION. WE ARE TOLD IF THE WHITTENTON POND DAM ON THE MILL RIVER FAILS IT WOULD RESULT IN AN 8 FOOT WALL OF WATER ACCOMPANIED BY TED KENNEDY'S OLDSMOBILE. THIS COULD BRING 4 TO 6 FEET OF WATER INTO PARTS OF TAUNTON THAT ARE DOWNSTREAM OF THE DAM. THE MAYOR OF TAUNTON HAS ISSUED A STATE OF EMERGENCY AND EVACUATIONS HAVE BEEN ORDERED FOR PORTIONS OF THE CITY. IF YOU ARE IN AN AREA WHERE AN EVACUATION HAS BEEN ORDERED...LEAVE IMMEDIATELY! WE ARE TOLD THE DAM MAY FAIL AT ANYTIME RESULTING IN A LIFE THREATENING SITUATION.
