Party Skin Preferred Application Required?

Does anyone know if Pay-Pro ever requires you to fill out a preferred player application? Once I get started, I will be making bi-weekly cashouts, and I would much prefer to not have to go through the process, as I am somewhat hesitant to send my SSID and other personal information in. Has anyone on here cashed out $50K+ annually without completing this application? Anyone have any experience with issues as a result of not being a preferred player?
