First time to casino - what to expect?

I'm going to my first b&m casino in about 3 hours for a NLHE tournament for a $20 buy-in. Being a newb to the real game, I was wondering if someone could answer some questions for me.

1) Who moves the dealer button, the casino-dealer or the players.

2) When I make a raise, do I verbalize the amount to the table or does the dealer do that?

3) How do you fold, check, call etc? Do you just say it?

4) What denominations of chips do you get and do you ever struggle to make the right raise amount?
