Your preflop raise amount sucks. You need to get money in the pot when you know that you have a big equity edge. I suggest making it at least 12-15 to go preflop.

The preflop raise is too small after 2 limpers. You're out of position and all this raise will do is give information about your hand and build a big multiway pot that you get to play out of position. Fun!

The flop action doesn't make any sense. UTG+1 must've done something after you raised to $80 right?

After the flat call of a PSB and minraise by CO, you made a slightly-more-than-minimum raise yourself-- why?
