Default Tourney Structure

I am searching for some advice about the best way to structure a multi-table tourney. The following is a list of some (but certainly not all) of the questions I have regarding the tourney set-up. I would be eternally grateful if you could provide an answer to one or all of my questions. I would also very much appreciate any advice you could give me (whether it relates to one of my questions or not) about running a multi-table tourney?

-What is a good amount of chips for each player to start
with?

-In a rebuy/add-on tourney: should a player have the ability
to rebuy when his stack dips below a certain amount or
only when he gets busted out? Also, when a player rebuys
or adds on, should he receive a stck of chips equal to
that which he received to begin the tourney or should he
get more?

-How often should the blinds increase and would you recomend
including an ante after a couple rounds?

-What is a good ratio for the number entrants vs. the number
of places that get paid? And what percentage of the total
prize pool should each of the top finishers receive?
