I've read it's very common to be attacked from the side by one of your opponent's buddies rather than the guy you're confronting. It's hard to be on guard for because apparently adrenalin causes tunnel vision.

I got sucker punched once by a guy who was with about 8 of his friends and I was by myself. Luckily I was in my car and I hauled ass. I found him the next day by himself, and the results were a little different.
