At the start of the you deal out the doctor and specialist cards so everyone gets the same amount (keep them seperate) discard the odd cards.
When it's your turn you take the doctor card off the top of your pile. Do the operation it says. If you do it you get the money.
If you screw up everyone looks at their specialist cards and the person with the card that matches the botched operation gets to try it for the specialist fee.
A rule most people don't know is the leg one with the elastic. You don't hook it on the posts at the start and try to remove it. You put it in the hole and the operation is to put it on the posts.
Ahh...the joys of parenthood.
