Default AQs - how would you play?

Hi all. Thanks for the enlightening posts as usual. Please comment on the following hand, questions following the hand.

The first caller has been loose and the second caller is new to the table.

Questions:

1) Preflop: would you raise more? Or is this about right?
2) Flop: Do you lead out with a continuation bet? What size?
3) When raised back like this, what do you do in this situation?
