Unless the dealer had just sat down within the last 3-4 hands. Thata where the blame goes partially too. During the first few hands it's natural a dealer will not see who's got what in odd or large chips. But after a few hands then the dealer should notice and stop the action till that chip gets in the pot. Except however when that chip has been hidden behind the stacks and they just had no chance to see it. HOWEVER, on the push, the previous dealer should also let the incoming dealer know "seat 4 has 1k chip".
As for the floor....he's weak...kick him in the nuts.
