The stud/8 chapter in Hellmuth's book isn't that bad, but the advice you cite is off the mark. It is usually best to fold such hands for one bet, let alone two. As for your fifth-street question, the pot is rarely so large on fifth that you should be considering calling a bet with hands as weak as that. You're not calling raises when you catch bananas on fourth, are you?
