Thanks for the replies, all. I know its not difficult for any of the 3 setups I mentioned ($2, $3, or $4 blind) as making $1 of change is not a difficult proposition; I just wondered if there was a standard way of doing it. Its always seemed weird to me that 3/6 has a $1 small blind but 15/30 has a $10 small blind (I assume those are both standard? I've never seen them live any other way).

I don't believe any way is better than any other way; there are just adjustments that need to be made. I do, however, think that Foxwoods would be better off with a 6/12 game than the 5/10 kill (kill at a pot of $100, not a kill when someone wins two hands in a row) they currently spread. Most of my reasons for that have been discussed in other threads about whether or not 5/10 is a good format for live play.
