I played in a tourney at the Trop in AC, and this happened at my buddy's table. A guy bet post-flop and a lady called, and everyone else folded. The dealer said that the hand was head's up (you know- 1 on 1). The lady thought that meant the guy was all in, so she flipped her cards. She showed two pair.

They let her finish the hand, which she lost big-time, because the guy had top set and knew exactly what she had. After the hand was completed, she had been assessed a 10-minute penalty where she was anted and blinded out.
