Default Badly played - when should I have bailed out

Now that I have 12,000 6-max hands I am looking back at some of my bigger loses.
Some of them are obviously BAD play the main question is when and where did I go wrong.
I will give the hand up to the point where I think I went badly wrong.

Party Poker No-Limit Hold'em, $ Hero (6 max, 6 handed) converter

Hero ($59.65)
UTG ($56.95)
MP ($12.95)
CO ($187.42)
Button ($25.95)
SB ($40.2)

Preflop: Hero is BB with 9[img]/images/graemlins/diamond.gif[/img], 9[img]/images/graemlins/spade.gif[/img]. SB posts a blind of $0.25.
UTG calls $0.50, MP calls $0.50, <font color="#666666">3 folds</font>, Hero checks.

Flop: ($1.75) 9[img]/images/graemlins/heart.gif[/img], Q[img]/images/graemlins/diamond.gif[/img], T[img]/images/graemlins/club.gif[/img] <font color="#0000FF">(3 players)</font>
<font color="#CC3333">Hero bets $1.5</font>, UTG calls $1.50, <font color="#CC3333">MP raises to $3</font>, <font color="#CC3333">Hero raises to $8</font>, <font color="#CC3333">UTG raises to $15</font>, MP calls $9.45 (All-In), Hero ?

Reads are hard to post but I will give the PT stats:

UTG is LAG - 63%/5%/2.53
MP - no real stats
