Default Limit Holdem: raising all ins & HU caps - very basic stuff

Ok, I've been playing live in AC for a while now, and I thought I understood all this stuff. But last weekend I saw a few things which lead me to question my grasp of some of the basics.

These questions apply to live casinos, specifically to the Taj Mahal and the Borgota (the nice thing about Party Poker is that all these details are taken care of).

So anyway, here's question number 1. When can you raise someone who's all in?

For example, assume a $3/$6 game, 4th street, 3 players. What if UTG opens all in for $1 and I'm next to act? What are my options? Can I only call or complete, or can I bring it up to $7?

What if he opens all in for $4 (more than 1/2 the bet). Now can I either call, complete, or raise to $10?

What if someone opens for $6 ahead of him, and he goes all in for $8? What can I do? What if he goes all in for $10?

Okay, second question. Capping. Both the Taj and Borgata have rules that say if it's heads up, there's no cap on the number of raises. Easy enough. There's supposedly an exception, however, that if there are three people in for 3-bets, then the usual cap applies (ie even if there are only two left, four bet max). Could someone explain this to me?

Also, I'm going out to Vegas in January. Are these rules pretty standard over there ?
