I think part of the assumption that the ICM is approximated by your chip equity is that you will still be playing poker. If you had the sit out button on for instance you ICM would be lower than...

I am not good enough to fold kings preflop-ever. I would consider it, if my opponent showed me aces, but I would probably call anyway for consistency /images/graemlins/cool.gif.

I would give him incorrect odds to call with an ace. That gives him the chance to make a mistake. All in gives him an easier decision

I make a 1/2 pot bet to keep two pair and pair plus straight draws around.
I am prepaired to get my stack in against a check-raise.

I would bet the pot, you are probably ahead, and he will likely call if he got a piece of the flop, if not you are not getting any more money out of him
