Check the PT help files for PokerRoom HH files. On EB, they are written to a .db file called YourNameTexasReal, and you have to point PT in that direction.

Do a search. Through bet365 you have to do a merchant transfer. A search should provide their merchantID, which I don't remember. Quickest withdrawal I've ever received.
